We are moving to a new filing system and want to update our current records, so we will be requiring everything listed below to be updated yearly to ensure we have correct contact information, pet information, and our Pet Parents are updated on policies.

Grooming Policies & Agreement – Grooming Clients Only

We want to provide your pet with the best and safest experience possible and as such we do require certain documentation/paperwork to stay informed of vaccinations and behaviors. We will be updating our records annually as of January 1, 2020 so any missing paperwork will be required whether the Pet Parent has been with us for years or it’s their first visit. If you would like to speed up your next check in please take the time to download, read, and complete any required paperwork below and new pet parents ensure our first visit checklist is completed. Grooming Intake PacketBoarding Intake Packet
The Grooming Intake Packet covers the Pet Parent’s & one (1) pet. Additional Pet Profile’s and Grooming Profile’s will be required for multi-pet families.

The Grooming Policies & Agreement covers all Policies enforced by Pawsitively Purrfect upon our Grooming Clients (This is included in Grooming Intake Packet)
Pet Grooming ProfilePet Boarding Profile required one (1) per pet
The Pet Grooming Profile covers the pet’s history with Grooming and general questions regarding their Grooming service
Pet Profile required one (1) per pet
The Pet Profile briefly covers the pet’s personality, medical history, and medications (This is updated yearly)
Pet Parent Profile one (1) per pet
The Pet Profile briefly covers the pet’s personality, medical history, and medications (This is updated yearly)
Senior Release required for each pet over the age of 10 years old
The Senior Pet Release cover’s what actions you would like taken if an medical emergency arose from your pet’s health/age.
